Today’s email is to ensure our peers, students, physical therapists and assistants are aware of the recent mandate issued by the governor regarding vaccination requirements by October 1st.

We understand that some members and clinics have questions, including the potential impact on patient access. We are working closely with our lobbyists at Mayforth Group, as well as state and government affairs staff at APTA.


In alignment with APTA, our chapter encourages PT’s, PTA’s and students to get vaccinated.  APTA, AOTA, and ASHA recently issued a joint statement on COVID vaccinations that can be viewed at: News | AOTA, APTA, and ASHA Joint Statement and Call for Professional Action on COVID-19 Vaccination and Education | APTA

Outpatient Provider Advisory         January 15, 2021


To Outpatient Providers:

In an effort to efficiently capture the entire outpatient provider population for COVID-19 vaccine registration, a survey is being sent to all email addresses on file for licensed healthcare professionals with the Rhode Island Department of Health's (RIDOH) Center for Professional Boards and Licensing L2K database.

RIDOH is asking you to work with your colleagues to identify a single point of contact at your practice (e.g., office manager, vaccine point of contact, etc.). Please have this single point of contact fill out this survey and return it to RIDOH at by January 22, 2021.


  1. Please coordinate and only submit one survey per practice. (Include all practice sites in one survey.) 
  2. If you have been identified as the single point of contact for your practice, please identify all clinical and non-clinical staff who wish to be vaccinated and populate the survey with key details:
    • First name and last name as they appear on a photo ID;
    • Age range of each individual (65 or older, 40 – 64, younger than 40); and
    • Email address for each individual. (The process to register individuals for a COVID-19 vaccine in Rhode Island requires an email at this time. If the individual who wishes to be vaccinated does not have an email, they are encouraged to create one.)
  3. Return this survey to by January 22, 2021.

Do not complete this survey if your organization has already been given the chance to be vaccinated (e.g., hospital, first responder, etc.). If specific individuals at your organization have already been vaccinated or have an appointment scheduled, do not include them in the survey.

Responses to this survey will serve as the basis of identification of clinical and non-clinical outpatient staff. Registration links to sign up for vaccination appointments will be sent via email regularly to the outpatient population identified by the survey, for vaccine clinics that will begin the week of January 25, 2021. This list will be used to send COVID-19 vaccination registration links to all outpatient provider staff who desire to be vaccinated. The information from the survey will serve to verify that individuals who arrive at vaccination sites were identified as ‘outpatient providers’ by their employer.
